Our value-based care model puts patients at the center of everything we do. It is thoughtfully and intentionally tailored to meet the specific needs of our adult patients ages 65 and older.
Our approach allows our physicians to deliver high-quality, personalized care and achieve better outcomes, while also focusing on health equity. And with the support of an interdisciplinary team that include care coaches, pharmacists and behavioral health specialists, our doctors have more time to build meaningful relationships with patients.
